On the Right Track Session #2

Session #2 - Team Work!: Preparing for the transitioning process together (open to parents with their youth ages 14-17 yrs). Session will cover planning and goal setting, summarizing your medical history and creating a health passport.Come meet other families with Inflammatory Bowel Disease! Beverages and snacks will be provided. If you have not done so already, please arrive approx. 15 minutes early, with your parent/guardian, to sign consent forms prior to participating in the session.
